SBH is seeking a qualified (RN) who will be responsible for providing and coordinating high-quality nursing care for individuals residing in the Intermediate Care Facility (ICF). The RN promotes optimal health, safety, and quality of life by assessing residents’ needs, developing and implementing individualized care plans, collaborating with interdisciplinary team members, and ensuring compliance with all federal, state, and facility regulations.

Responsibilities
  • Review the individual’s medical history and assess the need for nursing and medical activities.
  • Interview and counsel the individual and/or care providers regarding nursing and medical needs.
  • Record pertinent individual information in the individual’s case record.
  • Confer and share information with other members of the individual’s treatment team.
  • Evaluate and establish appropriate treatment programs designed to meet the needs of the individual.
  • Establish realistic/measurable goals for the individual and their care providers.
  • Share specialized knowledge/training with Hope House staff and other members of the treatment team.
  • Sign off on all medication changes within the individual’s case record and alert the pharmacy of such changes.
  • Participate in designated treatment team meetings.
  • Administer medications and treatments in accordance with physician orders and nursing standards.
  • Coordinate physician appointments, laboratory testing, diagnostic services, and specialty consultations.
Qualifications
  • Licensed by the state of Virginia as a Registered Nurse.
  • Experience practicing as a Registered Nurse, preferably with experience related to individuals with intellectual disabilities and/or experience in an intermediate care facility, environment.
  • Ability to supervise and lead nursing personnel effectively.
  • Proficient in electronic health records (EHR) and documentation systems.
  • Must meet all criminal background and drug screening requirements.
  • Valid driver’s license and safe driving record, required.
