Provides medically necessary behavioral health case management and individualized support services for adults diagnosed with serious mental illness and/or co‑occurring disorders. Work is performed independently within well‑defined guidelines under the direction of the Integrated Behavioral Healthcare Manager.

• Coordinate services with Spokane RSA behavioral health agencies and providers for individuals enrolled in primary behavioral health care services.
• Provide direct behavioral health services to individuals enrolled in division programs.
• Participate in intake interviews, complete intake documentation, develop individual treatment plans, and complete required forms.
• Assess individuals for program eligibility and collect necessary documentation for housing programs.
• Design person‑centered treatment plans that emphasize strengths and set recovery goals.
• Assist individuals in reducing isolation, depression, and other mental health symptoms.
• Conduct housing searches, secure placement in safe housing, and advocate to property managers and the Fair Housing Alliance regarding discrimination.
• Navigate HUD paperwork for housing access for chronically homeless, low‑income, disabled families.
• Orchestrate lease signing, condition reports, moving services, and establishment of utility services.
• Maintain stabilization and community integration after housing goals are achieved.
• Assist in maintaining housing arrangements, resolving crisis situations, and mediating tenancy problems with property management.
• Collaborate with local governmental agencies (DOH, DMV, SSA, Immigration, HUD, Spokane Housing Authority) to secure additional resources.
• Determine and coordinate community resources that support treatment goals.
• Educate on daily living activities and connect clients to health, dental, vocational, legal, money‑management, food‑bank, and other community services.
• Provide transportation for housing searches and community events.
• Assess threats, felony charges, and weapon possession to ensure safety of individuals and housing providers, and develop safety plans or contact crisis lines as needed.
• Coordinate the Community Centralized Individual/Family intake process for placement in supportive housing programs.
• Coordinate Community Integration Program social events.
• Ensure compliance with contract requirements, productivity standards, documentation, and medical‑necessity criteria.
• Utilize the Electronic Medical Record for timely documentation and audit files for accuracy.
• Participate in system‑usage meetings and coordinate behavioral health issues with MCOs and partners.
• Assess individuals for risk to self or others and provide referrals or safety plans.
• Follow federal and state confidentiality and security protocols.
• Attend staff and agency meetings and communicate regularly with leadership.
• Complete required training in WAC, RCW, and clinical skill development.
• Encourage community networking and support treatment team meetings.
• Liaise between outpatient providers and MCOs to facilitate resources and address barriers.
• Provide guidance to individuals and providers to align with the CSD mission.
• Identify solutions to non‑standard requests and maintain collaborative relationships.

• Bachelor’s degree in Counseling or a related social science, or an Associate’s degree in a human services field with two (2) years of behavioral health experience.
• OR meet DOH Registered Agency Affiliated Counselor (AAC) criteria with at least five (5) years of related experience, including student intern, juvenile rehabilitation counselor, or peer counselor experience.
• Possession or ability to obtain an AAC credential within 60 days of hire.
• Obtain National Provider Identifier (NPI) within 30 days of hire and maintain it.
• Valid driver’s license at time of hire and for the duration of employment.
