Developmental Disabilities (DD) Services Intake Specialist (ID078)

The Developmental Disabilities Services Intake Specialist is responsible for coordinating the assessment and intake process to individuals with developmental disabilities and their families while ensuring compliance with state and Agency guidelines. This role involves developing strong relationships with community partners, conducting comprehensive eligibility assessments, and providing support and resources for individuals on the DD waiver waitlist. Key responsibilities include evaluating individuals for diagnostic and functional eligibility, determining priority levels, and maintaining accurate records in the Virginia Waiver Management System.

The position also requires ongoing engagement with individuals and families, offering education, resources, and crisis intervention as needed. This role involves collaboration with internal and external stakeholders to address service gaps, facilitate referrals for case management, and provide excellent customer service. Additional duties include outreach activities and serving as a liaison on the Project Search steering committee.

MAJOR

DUTIES AND RESPONSIBILITIES:

  • Develops and maintains good working relationships with community partners, Agency staff, and other resources to ensure effective coordination of services.
  • Assesses individuals for Developmental Disabilities services by:
    • establishing diagnostic eligibility by determining if the diagnosis meets the state definition for a qualifying disability.
    • completing the Virginia Individual Developmental Disability Eligibility Survey (VIDES) to determine functional eligibility.
    • determining priority level according to state guidelines.
    • completing the Critical Needs Summary, when required.
    • placing them on the DD waiver waitlist through entering required information in the Virginia Waiver Management System.
  • Works closely with individuals seeking services, their families, other Agency staff, and other community stakeholders to provide education, guidance on the documentation process, necessary resources, such as for individuals in crisis situations, and to determine the gaps in information in order to provide excellent customer service.
  • Provides supportive intervention, necessary resources, and follow-up for individuals and families on the waitlist.
  • Monitors individuals on the wait list by providing annual contact by phone, mail, or in person.
  • Offers input and/or referrals for possible case management through identifying individuals with a higher priority need and refers to the DD Case Manager Supervisor and /DD Program Manager.
  • Provides DD services in compliance with the Department of Medical Assistance (DMAS) and the Department of Behavioral Health and Developmental Services (DBHDS) licensure guidelines.
  • Conducts outreach efforts, creates materials for and serves as the Agency spokesperson for DD Services at resource fairs. Attends IEP meetings in relation to DD Services, as needed.
  • Conducts educational presentations regarding DD Services to Agency employees and community stakeholders, as needed.
  • Serves as the liaison for the Agency on the Project Search steering committee.
  • Answers and responds to DD services phone calls as the primary phone contact in the DD program.
  • Attends Agency and DBHDS mandatory trainings.
  • Performs other duties as assigned.

QUALIFICATIONS:

  • A combination of work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
  • Bachelor’s degree in a relevant human services field required.
  • At least five years of experience working with DD individuals.
  • Experience with entering information into a database, experience with WaMS preferred.
  • Must possess a valid driver’s license, a good driving record, and the ability to provide your own transportation.
