Moms In Motion / At Home Your Way is seeking a Service Facilitator Case Manager to visit clients in their homes and guide families through Virginia’s Medicaid Waiver programs. You will write plans of care and assess needs to help families live safe, healthy, and independent lives.
Two years of relevant experience are preferred, but no degree is required. Strong communication and tech skills are essential, along with a valid driver’s license and the ability to pass background checks.
