Position: Nurse Practitioner (Primary Care)

Summary

Nurse Practitioner - Primary Care This position will support the Central Virginia Health Care System. The Nurse Practitioner is a licensed independent practitioner who is responsible for managing health problems and coordinating acute and chronic health care for a wide range of patient populations in accordance with State and Federal rules and regulations and the nursing standards of care.

Duties
  • Scope of Practice and Functions:
    The incumbent is responsible and accountable for provision of care for a specific group of patients to include the management of complex patient care.
  • He/she will provide leadership in the application of the nursing processes to patient care - organizational processes and/or systems improving outcomes at the program or service level.
  • He/she will be responsible for activities considered to be usual nursing practice and selected expanded functional defined in approved protocols and clinical practice guidelines.
  • He/she is involved in direct patient care - patient and family teaching - staff education and multidisciplinary consultations and collaborations.
  • Other duties includes but are not limited to:
    Works as a member of a teamlet within Primary Care PACT teams or Employee/Occupational health to ensure the highest quality of care is offered to Veterans enrolled in the system and within Employee/Occupational health the Medical Centers employees Participate in all Primary Care staff meetings - team huddles - etc.
  • Maintains recognized Nurse Practitioner standards of Practice and follows approved Scope of Practice.
  • Exhibits strong clinical skills to improve patients care for an aging - adult population.
  • as well as excellent interpersonal relationship skills in interpreting and communicating health care information to the patient - family and that interdisciplinary health care team.
  • Provides comprehensive healthcare which includes the assessment - diagnosis - planning - ordering - and interpretation of therapeutic interventions to a population of female Veterans across the lifespan.
  • Provide preventive health education - gender specific sexuality and mental health counseling - and referrals as needed.
  • Determines the need for a variety of diagnostics modalities to obtain health data on the case.
  • interprets results and make diagnoses of diseases and/or disorders.prescribe evidenced-based pharmacologic and non-pharmacologic interventions - to include referral when appropriate.
  • Provides gender specific examinations PRACTICE:
    Applies the nursing process to systems or processes at the unit/team/work group level to improve care.
  • Demonstrates leadership by involving others in improving care.
  • Performs an advanced - comprehensive - and holistic health assessment - including a relevant health history and physical assessment.
  • Determines the need for - orders - performs - receives - and interprets the appropriate diagnostic tests.
  • Analyzes and synthesizes data from multiple sources to establish a differential and working diagnosis.
  • Documents appropriately Veteran's history and physical - as well as requesting appropriate consults ETHICS:
    Supports and enhances client self-determination.
  • Serves as a resource for client and staff in addressing ethical issues.
  • Maintains confidentiality of all documents - patient reviews - and external reviews.
  • Serves as a resource for patients and staff in addressing ethical issues.
  • Initiates Veteran and/or family/significant other education on advance directives and end-of-life care issues.

Monday-Friday - 8:00am-4:30pm
