Nurse Practitioner - Primary Care

This position will support the Central Virginia Health Care System. The Nurse Practitioner is a licensed independent practitioner who is responsible for managing health problems and coordinating acute and chronic health care for a wide range of patient populations in accordance with State and Federal rules and regulations and the nursing standards of care.

Scope of Practice and Functions:

The incumbent is responsible and accountable for provision of care for a specific group of patients to include the management of complex patient care. He/she will provide leadership in the application of the nursing processes to patient care, organizational processes and/or systems improving outcomes at the program or service level. He/she will be responsible for activities considered to be usual nursing practice and selected expanded functional defined in approved protocols and clinical practice guidelines.

He/she is involved in direct patient care, patient and family teaching, staff education and multidisciplinary consultations and collaborations.
