Pay Rate: $38.00–$39.00 per hour (DOE)

Schedule: Part-Time

Concord Post Acute is seeking a dependable, compassionate, and skilled Licensed Vocational Nurse (LVN) to join our nursing team on a part-time basis. The LVN will provide direct nursing care to residents, support clinical staff, and help ensure exceptional patient outcomes in a fast-paced post-acute care environment. This role requires strong clinical judgment, excellent communication skills, and a commitment to delivering high‑quality resident‑centered care.

• Provide direct patient care in accordance with established nursing standards, physician orders, and individualized care plans.
• Administer medications, treatments, and nursing procedures as prescribed.
• Monitor resident conditions and accurately document assessments, interventions, and outcomes.
• Promptly report changes in resident status to the Charge Nurse, Registered Nurse, or attending physician.
• Assist with admissions, discharges, and transfers of residents.
• Collaborate with physicians, nurses, therapists, and other interdisciplinary team members to ensure continuity and quality of care.
• Maintain accurate, timely, and compliant documentation in accordance with facility policies and regulatory requirements.
• Promote a safe, clean, and supportive environment for residents, families, and staff.
• Respond effectively to changing resident needs and prioritize care appropriately.
• Adhere to infection control, safety, and quality assurance standards.

• Current and active California Licensed Vocational Nurse (LVN) license.
• Current CPR/BLS certification.
• Minimum one (1) year of experience in a skilled nursing facility, rehabilitation center, or post-acute care setting preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently while functioning effectively as part of a multidisciplinary team.
• Commitment to providing compassionate, resident‑focused care.

• Must be available to work weekends, holidays, and variable shifts as needed.
