Nurse Practitioner or Physician Assistant - PRN

Job Summary

Matrix Medical Network is an independent provider of comprehensive health assessments performed at members’ homes or skilled nursing facilities. We seek Nurse Practitioners to conduct adult and geriatric comprehensive health assessments (CHA) and work collaboratively with physicians, case managers, and other care team members.

During a visit that can last up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well‑being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps improve quality of care and close multiple care gaps with a single visit.

• Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up and clear documentation per Matrix guidelines and protocols.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physician (PCP) on patient education and follow‑up as necessary.
• Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities.

• Flexible scheduling – visits scheduled Sunday‑Saturday from 8 am–7 pm based on your availability.
• Competitive pay – earn income based on the number of assessments completed. Monthly earnings range $1,800–$11,500 depending on per‑assessment rate $90–$115.

• Master’s Degree or equivalent experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice, including prescription authority or ability to obtain prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred; new graduates encouraged to apply.
• Valid state driver’s license and automobile insurance coverage (or ability to obtain).
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

• State‑specific insurance requirements:
  Bodily Injury $100,000 per person / $300,000 per incident;
  Property Damage $50,000.
• Collision and comprehensive deductibles $1,000 or less.
