• Conduct adult or geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case‑management referrals, follow‑up, and clear documentation according to Matrix guidelines and protocols.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with a Primary Care Physician on patient education and follow‑up as necessary.
• Provide services at the member’s place of residence, which may be their home or a Skilled Nursing Facility.

Flexibility: Visits are scheduled based on your availability. Sessions run Sunday‑Saturday from 8 am to 7 pm, with options for day, evening, or weekend work.

Competitive Pay: Earn income based on the number of assessments completed. Monthly earnings range from $1,800 to $11,500 (20–100 visits) with a per‑assessment rate of $90–$115 depending on state.

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• One year of NP experience preferred; new graduates encouraged to apply.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage required for the state of residence (or ability to obtain).
• Insurance requirement:
  Bodily Injury – $100,000 per person/$300,000 per incident;
  Property Damage – $50,000;
  Collision and comprehensive deductibles – $1,000 or less.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
