Nurse Practitioner or Physician Assistant; PRN - In-Home Health Assessments

Overview

Matrix Medical Network seeks Nurse Practitioners to provide comprehensive, in‑home health assessments for older adults and other at‑risk individuals across the U.S. The role involves conducting up to one‑hour visits at member residences or skilled nursing facilities.

• Conduct adult/geriatric assessments, including patient history, diagnosis, treatment plan, health education, physician and case‑management referrals, and clear documentation per Matrix guidelines.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
• Coordinate with the primary care physician (PCP) on patient education and follow‑up.
• Provide services at the member’s home or skilled nursing facility.

• Master’s degree or equivalent and successful completion of Nurse Practitioner licensure.
• Current NP licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority.
• Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialties.
• Current BLS, ACLS, or CPR certification.
• One year of NP experience preferred; new graduates encouraged.
• Valid state driver’s license for the state of residence and proof of automobile insurance coverage.
• Liability insurance: body injury $100,000 per person / $300,000 per incident; property damage $50,000; collision and comprehensive deductibles $1,000 or less.
• Strong computer skills and familiarity with electronic health record software.
• Excellent verbal and written communication skills.
