Nurse Practitioner, Geriatric Nurse Practitioner

Overview
Sign on Bonus: $2,000.
Responsibilities- Conduct adult and geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Administer and order point-of-care and laboratory testing as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
- Work with the Primary Care Physician on patient education and follow-up.
- Provide services at the member’s home or Skilled Nursing Facility.
- Master’s Degree or equivalent experience and satisfactory completion of NP licensure.
- Current NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority as needed.
- Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Valid BLS, ACLS, or CPR certification.
- One year of NP experience preferred; new graduates encouraged to apply.
- Valid state driver’s license and automobile insurance coverage for the state of residence.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
- Flexibility:
Schedule visits based on availability. Assessments are conducted Sunday to Saturday, 8 a.m. – 7 p.m.; work during day, evening, or weekend per your schedule. - Competitive Pay:
Earn income based on the number of assessments completed. Monthly earnings range from $1,800 to $11,500 for 20–100 visits, at $90–$115 per assessment (state-dependent). - Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.

Comprehensive onboarding, a preceptor, 1:1 feedback, and free access to continuing education. - Additional Licensure Support:
