About the Organization Connect Health + Wellness is committed to providing primary health, dental and integrated behavioral health services at our Federally Qualified Health Center sites and Dental sites by promoting health, reducing health risk factors and increasing access to medical and dental services, primarily for the uninsured and underserved in our service area. Additionally, we offer chronic disease self-management, medication assistance, and care coordination services to patients in our service area.

This position may be based out of our Ridgeway or Bassett, Virginia location.

Our Family Nurse Practitioners provide comprehensive, evidence-based, primary and preventive medical care in a team-based environment to a diverse patient population with a wide range of health conditions and concerns. Perform physical examinations and preventive health measures within prescribed guidelines and instructions of Physician.

This position will be on day shift and may possibly work three 12-hour shifts per week or four 9-hour shifts per week based on our current needs.

• Graduate of an approved and accredited school of nursing recognized by the Virginia State Board of Nurse Examiners with a nurse practitioner degree or certificate.
• Current, valid nurse practitioner’s license in the State of Virginia.
• Recognized by the Virginia Board of Nurse Examiners as an Advanced Practitioner.
• Current DEA license, and CPR and AED certifications.
• Prefer at least one year’s experience in a primary health care setting.
• Prefer bilingual in English/Spanish.
• Relates well to people of various socio-economic backgrounds.
• Someone who is well organized, enjoys multi-tasking and thrives in a high energy, fast-paced environment.
• Proficient with electronic medical records software programs.
• Evidence of involvement and willingness to participate in community leadership activities.
• A passion and concern for the target population we serve.

$95,000 to $120,000 annually
