Job Overview
Home Health Nurse – PRN/Flex (Open). Employment status:
Variable/Flex. Shift: Day (United States of America). Facility: 390 S Main St – Rocky Mount. Requisition Number: R153269.
- Assesses, plans, implements, and evaluates patient care to achieve optimum outcomes for patients of all ages.
- Completes age‑specific admission and ongoing assessments, documents findings, and develops a plan of care.
- Collaborates with the health‑care team, patients, and families to develop, implement, and evaluate an interdisciplinary plan of care; revises the plan as patient status changes.
- Accurately administers treatments and medications and documents them using automated computer systems, including electronic medical records.
- Identifies patient/family educational needs and incorporates them into the plan of care, implementing and documenting appropriately.
- Performs tasks within clinical, leadership, and education pathways to support evidence‑based, quality patient care and performance improvement.
- Demonstrates clinical expertise in rapidly changing circumstances, plans strategies to reduce length of stay, turnaround time, and resource consumption, and improves patient outcomes.
- Acts as a role model for clinical practice to coworkers, students, and physicians.
- Manages resources, establishes priorities, and modifies the environment to meet patient care needs.
- Completes manager/director‑assigned projects within specified time frames and according to guidelines.
- Uses problem‑solving methods to facilitate resolution of patient and personnel situations that arise in the work area.
- Participates actively in the orientation of new employees.
Interacts with patients, families, visitors, and all members of the health‑care team to develop, coordinate, and manage the plan of care. Delegates and oversees delivery of patient care while maintaining confidentiality. May be exposed to infectious diseases, bio‑hazardous and hazardous materials, and regulated medical waste. Work is fast‑paced and requires prioritization skills. Physical requirements include lifting, prolonged standing, bending, pushing, pulling, reaching, and walking.
May be required to provide care at other Carilion facilities and may need to take call.

Qualifications- Graduate of a professional nursing school.
- One year of experience in hospice, home health, oncology, or medical‑surgical nursing.
- Registered Nurse license in the Commonwealth of Virginia.
- Valid Virginia driver’s license and pass a Criminal Record Check (for Home Health).
- American Heart Association BLS‑HCP certification.
- Keyboarding skills required for electronic medical records.
- Must successfully complete orientation and competency requirements.
Career Advancement Program (CAP) is available to RNs to attain various levels of advancement. Levels are determined by achievements outlined in the CAP Scoring Sheet, part of the RN performance appraisal.
