About the Organization Connect Health + Wellness is committed to providing primary health, dental and integrated behavioral health services at our Federally Qualified Health Center sites and Dental sites by promoting health, reducing health risk factors and increasing access to medical and dental services, primarily for the uninsured and underserved in our service area. Additionally, we offer chronic disease self‑management, medication assistance, and care coordination services to patients in our service area.
DescriptionThis position may work out of Connect Health + Wellness Bassett or Ridgeway clinics but will support patients at each of our Primary Care sites.

Position SummaryThe Psychiatric Mental Health Nurse Practitioner (PMHNP) delivers compassionate, patient‑centered psychiatric care across the lifespan. This role involves collaboration within a multidisciplinary care team to assess, diagnose, and treat mental health conditions, create individualized treatment plans, and support patients in managing their mental wellness.
Key Responsibilities Patient Care- Perform comprehensive psychiatric assessments, including history, mental status exams, and diagnostic evaluations.
- Develop, implement, and revise individualized treatment plans.
- Prescribe and manage psychiatric medications in accordance with clinical guidelines and applicable regulations.
- Provide psychotherapy (individual, group, or family), as appropriate.
- Educate patients and families on diagnoses, treatment options, and preventive strategies.
- Collaborate with primary care providers, social workers, and other team members to deliver integrated care.
- Actively participate in team meetings to review and adjust patient care plans.
- Serve as a resource for staff regarding behavioral health concerns.
- Maintain accurate, thorough, and timely documentation in the electronic health record (EHR) system in accordance with organizational and regulatory standards.
- Engage in continuous quality improvement efforts to enhance clinical outcomes and service delivery.
- Stay informed on current best practices and evolving trends in psychiatric care.
- Support mental health education and outreach efforts to reduce stigma and promote awareness.
- Collaborate with community organizations to improve access to behavioral health resources.
- Perform additional duties as assigned by the supervisor that are aligned with the mission and scope of this role.

Pay Range: $96,000 to $120,000 annually
Position Requirements Qualifications Education- Master’s or Doctoral degree in Nursing from an accredited institution, with a concentration in psychiatric‑mental health nursing.
- Current, unrestricted Nurse Practitioner license in the Commonwealth of Virginia.
- Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) by the American Nurses Credentialing Center (ANCC).
- Minimum of two (2) years of experience in psychiatric‑mental health care, preferably within a community health setting.
