Geriatric Nurse Practitioner, Nurse Practitioner

Overview
Matrix Medical Network is an independent provider of comprehensive health assessments. Sign‑On Bonus: $7,500.
Responsibilities- Conduct adult / geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Matrix guidelines.
- Administer and order point‑of‑care and lab‑based testing as needed.
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physician (PCP) on patient education and follow‑up as necessary.
- Provide services at the member’s place of residence (home or Skilled Nursing Facility).
Consistency:
Providers typically work Monday–Friday, 8:00 am–5:30 pm, with assessments available Sunday–Saturday 8 am–7 pm.
Competitive Pay:
Annual base salary ranging from $105,500 to $168,400, based on market compensation analysis and experience.
Incentive Program:
Eligible for the full‑time Clinician Traveler Incentive Plan with uncapped monthly incentive opportunity.

Master’s Degree or equivalent experience and satisfactory completion of NP or PA licensure.
Current NP/PA licensure in the state of practice with prescription authority or ability to obtain prescriptive authority.
NCCPA, AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
Current BLS, ACLS, or CPR certification.
Valid state driver’s license for residency state and automobile insurance coverage required.
Qualifications- One year of NP or PA experience.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
- Ability to travel up to 80–90 % of the time for extended periods, including overnight travel and weekends across the assigned state and possibly neighboring states.
Bodily Injury: $100,000 per person / $300,000 per incident.
Property Damage: $50,000.
Collision and comprehensive deductibles: $1,000 or less.
