As a Hospice RN Case Manager, you will play a vital role in coordinating comprehensive care for patients and families, ensuring high-quality hospice services while maintaining compassionate support.

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness according to care center policies.
• Document pain and symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Regularly communicate with the clinical manager and care team.
• Perform on‑call responsibilities and provide on‑call services to patients and families as assigned.
• Other duties as assigned.

• One year of nursing experience.
• Current RN license in the state(s) where you will be assigned.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.
