The Home Health and Hospice RN Case Manager assesses, plans, implements, and evaluates patient care to achieve optimum outcomes. This full‑time position is based in Rocky Mount, North Carolina, and involves independent patient visits with a focus on collaborative, team‑oriented care.

• Conduct age‑specific admission and ongoing assessments, document findings, and develop individualized care plans.
• Coordinate and manage the delivery of patient care across homes, ensuring confidentiality and adherence to departmental policies.
• Utilize delegation and critical‑thinking skills to provide high‑quality service, resolving routine issues independently and escalating complex matters to the Manager, Team Lead, or Supervisor.
• Maintain knowledge of laws and regulations related to CHCS and Carilion Clinic, interpreting them to guide patient care.
• Prioritize tasks in a fast‑paced environment, effectively multitask, and manage call responsibilities when required.
• Perform physical tasks such as lifting, standing, bending, and walking as needed for patient care.
• Support the Culture of Excellence and actively practice the Carilion Clinic Mission, Values, and Guiding Principles.

• Graduate of an accredited Registered Nurse Program.
• Currently licensed as a Registered Nurse in Virginia or an approved compact state.
• Valid driver’s license, automobile insurance, and personal vehicle for home visits.
• Proficient keyboarding skills for electronic medical record systems.
• Demonstrated knowledge of lifespan growth and development principles.
• Strong critical‑thinking skills and ability to interpret regulatory guidelines.
• Experience not required; entry‑level position.

• Competitive salary ($33.52 – $50.28 per hour).
