• Home Visits: Conduct regular, personalized visits to evaluate and support patients' needs.
• Comprehensive Assessments: Thoroughly assess physical, psycho-social, and environmental factors in the home.

• Collaborative Care: Partner with multidisciplinary healthcare teams to ensure seamless care delivery.
• Documentation & Supervision: Maintain timely and accurate visit documentation, and provide guidance to home care aides.

• Licensure: Active Registered Nurse (RN) license in good standing (Virginia state).
• Education: Graduate of an accredited nursing program (Associate, or Bachelor’s Degree).
• Certifications: Current CPR / BLS certification.
• Experience: 1 year of RN home health/home care experience preferred.
• Requirements: Valid driver’s license, reliable transportation, and active auto insurance.
• Core Competencies: Strong clinical judgment, excellent communication skills, and a compassionate, patient-first mindset.
