Personalized Home Visits: Perform health evaluations and deliver individualized care in patient homes.
Holistic Assessments: Evaluate physical, psycho-social, and home environment factors to tailor care plans.
Empower & Educate: Teach patients and family caregivers how to manage health conditions effectively.
Team
Collaboration:
Work alongside multidisciplinary teams to ensure complete, continuous care.Documentation & Leadership: Complete timely visit reports and provide supportive guidance to home care aides.
License: Active RN license in good standing (Virginia state).
Education: Graduate of an accredited nursing program (ADN or BSN).
Certifications: Active CPR / BLS certification.
Experience: 1+ years of home health or home care experience preferred.
Travel Requirements: Valid driver’s license, reliable vehicle, and active auto insurance.
Skills: Strong clinical judgment, exceptional communication, and a patient-first heart.
