We urgently needed the experienced Licensed Practical Nurse (LPN) to join our team and make a meaningful impact on patient lives.

LPN Job Qualifications
  • Active LPN License (Licensed Practical Nurse) is required.

  • Minimum of 1 years of nursing experience preferred (home health, clinic, facility, hospice or related setting).

  • Experience working with patients with complex medical needs

  • Reliable, compassionate and professional work ethic.

  • Experience with electronic medical records.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and as part of a team.

  • Ability to create reports.

LPN

Job Responsibilities
  • Provide direct nursing care as a Licensed Practical Nurse (LPN) in client’s homes.

  • Develop and implement individualized care plans.

  • Administer medications and treatments as prescribed by physicians.

  • Monitor and record patient vital signs and medical history.

  • Communicate regularly with physicians, family members, and other healthcare professionals.

  • Writing a comprehensive daily report.
