We urgently needed the experienced Licensed Practical Nurse (LPN) to join our team and make a meaningful impact on patient lives.

We are looking for a Full time and Part time LPN to care and assist our client, Monday to Friday, 9AM-5PM.
Supportive and collaborative Home Care team focused on providing quality patient home care in Virginia.

Active LPN License (Licensed Practical Nurse) is required.
Minimum of 1 years of nursing experience preferred (home health, clinic, facility, hospice or related setting).
Experience working with patients with complex medical needs
Reliable, compassionate and professional work ethic.
Experience with electronic medical records.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Ability to create reports.
Job Responsibilities
Provide direct nursing care as a Licensed Practical Nurse (LPN) in client’s homes.
Develop and implement individualized care plans.
Administer medications and treatments as prescribed by physicians.
Monitor and record patient vital signs and medical history.
Communicate regularly with physicians, family members, and other healthcare professionals.
Writing a comprehensive daily report.
