Program: Journey of Hope Long Term Foster Care (LTFC)

Reports to: Manager of Case Management Services

Location: Moon Township, PA

Hours: Full-Time position, exempt

ESSENTIAL FUNCTIONS

The Case Manager is responsible for coordinating case management services. The Case Manager’s tasks shall include development of intake assessment, individual service plan, family reunification and discharge. The Case Manager shall also ensure that all services provided to each child are properly documented in each child’s case file. Final approval for hire will be done by the Project Officer of the Office of Refugee and Resettlement, which is a sub‑agency of the Department of Health and Human Services of the federal government.

KEY RESPONSIBILITIES
  • Demonstrates leadership by promoting agency mission and professionalism, fostering teamwork, making sound decisions, and handling conflict and crises constructively.
  • Builds and maintains strong relationships with staff, clients, sponsors, and community stakeholders while supporting a collaborative team environment.
  • Ensures timely, accurate documentation, including intake assessments, case reviews, progress reports, referrals, and compliance with ORR/DPW requirements.
  • Manages family reunification process, including identifying sponsors, maintaining weekly communication, assessing readiness, preparing cases, and coordinating safe releases.
  • Coordinates and contributes to Individual Service Plans (ISP) by collaborating with clinicians and multidisciplinary teams to ensure appropriate services and track progress.
  • Maintains consistent communication with sponsors and minors, coordinating visits, facilitating calls, gathering input and supporting reunification efforts.
  • Facilitates service coordination, including referrals, collaboration with external providers, and ensuring minors’ engagement in program and community activities.
  • Supports discharge planning, including safety planning, medical coordination, referrals to post‑release services, and completion of discharge documentation.
  • Utilizes a team‑based approach, collaborating with youth care workers and interdisciplinary staff to address minors’ medical, emotional, and social needs.
  • Maintains regular direct contact with minors, monitoring progress, supporting goals, and ensuring compliance with ORR communication standards.
  • Complete and submit incident, Child Line, and CPS reports within mandated timelines, ensuring audit and compliance readiness.
  • Ensures compliance with policies and regulations, including HIPAA, ORR guidelines, and agency procedures while maintaining confidentiality.
  • Engages in professional development, including supervision, trainings (40+ hours annually), skill application, and meeting all job qualifications (education, bilingual ability, and clearances).
  • Completes other duties as assigned by supervisor.
REQUIREMENTS
  • Master’s degree preferred, or a bachelor’s degree in behavioral sciences, human services, or a related social service field.
  • 3–5 years of progressive experience in human services.
  • Must have Act 33/34 and FBI clearances and valid PA driver’s license.
  • Strongly encouraged to have experience in child welfare and/or case management.
  • Required clearances:
    Act 33, Act 34, and FBI clearances.
  • Must be at least 21 years of age.
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards.
  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
  • Ability to lift or push a minimum of 20 lbs. by themselves or up to 50 lbs. with the assistance of other.
