Moms In Motion is looking for a Service Facilitator (SF) Case Manager in Virginia. You will visit clients in their homes, help families navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and write care plans that improve safety, health, and independence.
Candidates should have at least 2 years of experience supporting individuals with disabilities or the elderly, no degree required, and a valid driver's license with a reliable vehicle.
