Location: Victoria
Moms In Motion is seeking a compassionate Service Facilitator (SF) Case Manager to serve Virginia families. You will visit client homes, help families navigate Virginia Medicaid Waiver programs, and support independence and safety in the home.
You’ll hit the road locally, coordinate plans of care, and troubleshoot authorizations and timesheets, all while building trustful relationships with families. No degree required; two years of relevant experience preferred, and strong communication skills
