Patriot-Enterprises-LL seeks a Systems Navigator for EFMP to coordinate services for families with special needs, between military and civilian providers. You will conduct intake interviews, document findings, identify needs, and refer families to appropriate resources.
Travel will be required. You will maintain records per Army/DoD regulations, present EFMP briefings, build community networks, and support families to access essential services through coordinated care.
