Field Based Services Team Lead (609) — Visalia, CA
Responsibilities- Lead a multi-disciplinary behavioral health team, providing direct clinical support to team members and delivering mental health services to the target population.
- Under the general direction of the Clinical Manager with substantial latitude for independent judgment, deliver mental health services to individuals in crisis, conduct risk assessments, biopsychosocial assessments, treatment planning coordination, MSE exams, diagnose SMI disorders, determine levels of care, and coordinate with law enforcement, families, and other agencies to ensure client safety and continuity of care.
- In the absence of the Clinical Manager, assume selected administrative, procedural, and clinical responsibilities to ensure team operations run smoothly.
- Provide direct mental health crisis intervention and assessment services across the county, independently conducting biopsychosocial assessments, mental status exams, determining level of care, identifying crisis situations, completing risk assessments, safety plans, and providing individual therapy and psychosocial rehabilitative services to a complex caseload.
- Develop and implement trauma-informed, client-centered, culturally responsive, harm-reduction and recovery-based treatment plans that address client needs and reduce impairments; refer to appropriate follow-up care and assist with discharge and aftercare planning.
- Deliver psychotherapeutic interventions in line with professional and ethical standards; collaborate with families, law enforcement, healthcare providers, and other community agencies around safety and care coordination.
- Offer consultation and informal supervision to other Mental Health Clinicians (licensed and unlicensed). Support documentation and crisis response training for staff as directed.
- Maintain service records, logs, and other required documentation; contribute to audit preparation and performance reporting. Assist with policy/procedure development and ongoing quality assurance activities.
- Represent the agency in community presentations and trainings related to mental health and substance use issues. Perform other duties as assigned to support the overall functioning and success of the program.
- May encounter risk of working with persons with SMI (serious mental illness); respond to crises by calling local emergency personnel to help deescalate, using crisis intervention techniques, and consulting with a supervisor as soon as practically possible.
- Master's Degree in Social Work, Psychology, Counseling, or related behavioral health field
- Licensure/Certification: LPCC, LMFT, LCSW and BBS Registration
- Driver’s License:
Class C - 2 years of professional experience in mental health counseling, psychotherapy, and related social services in a public or certified private welfare or health agency; at least 1 year of experience with high-risk and high-acuity clients
- Working knowledge of community and hospital resources; proficiency with Windows-based applications (Word, Excel, Outlook)
- Familiarity with EHR systems containing PHI
- Ability to develop rapport with clients, de-escalate crises, assess danger to self/others, follow DSM-V diagnostic criteria, conduct MSE and CANS assessments, and document interactions to Medi-Cal standards
