Location:
Visalia, CA 93291, USA Compensation: $18 - $19 per hour
- Assist advancing client’s decision-making, productivity and participation in a range of daily activities.
- Facilitating the client’s inclusion in positive community experiences.
- Promoting independence in daily activities while being mindful of client’s intellectual and physical disabilities and assisting with meals, cooking, household maintenance, grooming and dressing, as assigned.
- Maintaining current knowledge of client’s needs.
- Safety:
Maintain inventory of forms, packets, supplies, including PPE. Act as local Safety Officer and member of the Adjoin Safety Committee. Track and facilitate required safety training and certification (First Aid/CPR). Complete safety orientation training for new hires. - Communication:
Maintain positive communications with supervisor, clients, landlords, employers, family members, Regional Center, D.O.R., and other community members. Document activity records, reports, incidents, and client evaluations accurately and timely. - Training:
Implement individual training and support programs under direction. Address challenging behaviors proactively and respectfully per plans and training. - Administration:
Documentation management, scanning/uploading/filing, mail/faxes/packages handling, and petty cash tracking as needed. - Management:
Manage safety matters in the office, arrange safety training, maintain office environment and supplies. - Planning:
Plan and participate in safety meetings, coordinate vaccinations/workers’ compensation administration, and safety drills; maintain safety postings.
- Education:
High School Diploma or Equivalent. - Experience:
18 years of age or older. Successful completion of pre-employment drug screen and background check. Physical evaluations may be required depending on duties and may include TB test and/or lift and transfer test. - Certification:
Certification in an approved First-Aid and CPR class; or successful completion within 45 days of hire. - License:
Insured, registered vehicle in safe working condition, and proof of current CA driver’s license. Acceptable DMV record. - Mobile:
Adjoin’s client case management platform requires a mobile device with reliable internet. Employee must have a device; reasonable reimbursement provided. - Schedule:
Direct Support Facilitator works as scheduled by Regional Manager; may be part or full time. Overtime may be required with prior approval. - Travel:
Ability to travel within counties served, approximately 75% of the time.
