Overview
We are now hiring for our Parlier, Dinuba and Visalia Health Centers.
A Family Nurse Practitioner passionate about community health and family medicine, with a focus on performing Annual Health Assessments.
A healthcare professional committed to working with underserved patient populations, motivated and mission‑driven, whose experience has prepared them to contribute to our ICARE values of integrity, compassion, advocacy, respect and excellence.
ResponsibilitiesConduct Annual Health Assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs and a comprehensive physical exam.
Perform evidence‑based practice screenings, including point‑of‑care testing as appropriate.
Identify diagnoses to be used in care management and active medical management for treatment.
Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of the assessment.
Communicate findings in the assessment that will be used to inform the patient’s primary care provider of potential gaps in care.
Identify urgent and emergent situations and intervene appropriately.
Educate patients on topics such as disease process, medication, and compliance.
Qualifications- Certified:
California Family Nurse Practitioner License - DEA Certificate
- BLS or ACLS certification
- Graduate of an accredited Family Nurse Practitioner Program
- National certification in primary care specialty
- Qualified to practice by certification in the State of California
Experienced FNPs, particularly those who have worked in Health Centers or completed family practice training, are preferred.
The salary wage range that UHC reasonably expects to pay for the position upon hire starts at $ a year plus bonus. Our competitive salaries are dependent on knowledge, skills and experience. The base pay does not include other forms of compensation and/or incentives.
