Hospice Registered Nurse

Full Time Visalia, CA, US

About the Role:

We are seeking a highly skilled and compassionate Registered Nurse to join our Hospice team in Visalia, CA. As a Registered Nurse in Hospice, you will be responsible for providing skilled nursing care to patients in their homes, ensuring their safety, comfort, and well‑being. You will work closely with our medical director, and other healthcare professionals to develop and implement individualized care plans for each patient, and provide education and support to their families.

Your ultimate goal will be to support patients and their families with dignity, comfort, and love.

Minimum Qualifications:

  • Active and unrestricted Registered Nurse license in the state of California
  • Minimum of 1 year of experience in Hospice preferred
  • Current CPR certification
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Responsibilities:

  • Perform initial and ongoing assessments to monitor patient condition and adjust care as needed
  • Develop and implement individualized care plans based on patients' conditions
  • Administer medications, treatments, and procedures as ordered
  • Monitor and document patients' progress and response to care
  • Provide education and support to patients and their families

Skills:

As a Registered Nurse in Hospice, you will utilize your clinical expertise, critical thinking, and problem‑solving skills on a daily basis. You will need to be proficient in administering medications, treatments, and procedures, as well as in assessing and documenting patients' health status and progress. Excellent communication and interpersonal skills are essential, as you will be working closely with patients, families, and other healthcare professionals.

You will also need to be able to work independently and as part of a team, and be comfortable with using technology and electronic medical records to manage patient care.
