Essential Duties and Responsibilities
  • Under the direct supervision of the Regional Program Director, the Youth SUD Counselor II (YSUDC) will provide SUD treatment, prevention, and case management to youth ages 13-18 years old.
  • Maintain a caseload of clients, facilitate evidence-based curriculum in a group setting, provide individual and family counseling, case management, and conduct school and home visits as needed.
  • Complete intakes, ASAM assessments, treatment plans, discharge plans, case management, and referrals.
  • Work independently and with a multi-disciplinary team, consulting on all cases with supervisor.
  • Provide services as needed at other Champions SUD Treatment Programs.
  • Complete intakes and eligibility for potential clientele and verify completeness and accuracy of documents.
  • Conduct intakes and assessments using the American Society for Addiction Medicine (ASAM) assessment.
  • Collaboratively develop and maintain individualized treatment plans with clients based on the ASAM assessment.
  • Facilitate SUD treatment of psycho educational groups.
  • Develop and provide discharge planning sessions as treatment concludes.
  • Maintain caseload and provide active, comprehensive case management ensuring all area needs (mental health, SUD, social, physical, family, financial, employment, etc.) are met.
  • Participate in staff meetings, addressing client needs, program issues, and assisting in integration of services.
  • Link clients to resources and coordinate services with other providers.
  • Refer clients to community support services when appropriate.
  • Integrate all service divisions of the program, acting as a bridge between disciplines such as mental health and Probation.
  • Determine appropriate services available through Champions programs or referral to community agencies.
  • Collaborate with Champions staff, referring parties, and service providers.
  • Develop and maintain cooperative work relationships with community agencies and partners, including client connections to outside agencies (court hearings, open CPS cases and requirements, etc.).
  • Serve as the main point of contact for all clients and relay client needs to appropriate channels.
  • Conduct urinalysis and breath testing as needed.
  • Conduct home visits and provide client transportation as needed.
  • Maintain program data entry and attendance tracking.
  • Ensure appropriate and comprehensive up-to-date documentation within an EHR system and hardcopy filing system.
  • Provide SUD treatment services at other Champions SUD treatment programs.
  • Participate in applicable meetings, trainings, conferences, etc. as required internally and externally.
  • Remain culturally humble and respectful in all client and staff interactions.
  • Adhere to client confidentiality requirements and standards, including updates of the shared Release of Information.
  • Perform various projects in conjunction with day‑to‑day activities.
  • Get along with diverse personalities, demonstrating maturity and flexibility.
  • Possess strong judgment, problem‑solving, and reasoning abilities.
  • Perform other duties as assigned.
Qualifications
  • SUD Counseling Certification from a State of California Accredited Certifying Board (or registered with the Board of Behavioral Sciences) and maintain throughout employment; OR registered by one of the DHCS-approved SUD counselors for certifying organizations and maintain registration.
  • Bachelor's degree in social work, psychology, or related field; OR associate's degree plus 1 year of relevant experience; and/or 2 years of relevant experience may be substituted.
  • Priority given to applicants with 1‑2 years of work experience in a counseling or treatment setting with clients struggling with substance use, mental health, and criminal justice involvement.
  • Valid California Class C Driver’s License, good safety record, reliable vehicle, proof of comprehensive auto insurance if driving personal vehicle.
  • Must be drug‑tested prior to hire and test negative.
  • Must pass background check (Live Scan Fingerprint - MANDATORY).
  • Must have TB clearance within 60 days prior to hire (MANDATORY).
