The Director of Integrated Care plays a critical role in the development of the Innovation Management Office and in advancing Axia’s innovation and growth strategy by building, launching, and scaling new service offerings across the network. The role sits at the intersection of strategy, operations, and clinical integration – owning initiatives from initial concept through sustainable execution, with accountability for patient outcomes, financial performance, and network adoption.

The Director will manage a portfolio of new and existing initiatives, build business cases, lead cross‑functional execution, and embed new offerings into core operations.

Scope and Responsibilities
  • End-to-End Program Ownership
    • Own complex cross-functional initiatives from ideation through launch and scale
    • Define scope, timelines, success metrics and governance to ensure program integrity
    • Own project roadmap independently
  • Stakeholder and relationship management
    • Build strong, productive relationships with internal partners (clinical, operations, finance, marketing) and external stakeholders
    • Align diverse stakeholders toward shared goals and outcomes
  • Financial accountability
    • Build and own financial models for new offerings
    • Maintain accountability for P&L performance and long-term sustainability
  • Change management and adoption
    • Lead organizational change to support adoption of new service lines
    • Drive excitement, alignment, and engagement around innovation across care centers
  • Data and performance management
    • Define KPIs and success metrics tied to patient outcomes, operational performance, and financial results
    • Track progress, course correct as needed, and communicate results clearly
  • Problem-solving and standardization
    • Translate complex challenges into scalable, repeatable solutions
    • Develop and refine playbooks and best practices to support consistent rollout across the network
  • General support
    • Contribute to strategic analyses, business cases, and ideation to support broader Integrated Care priorities
Success Metrics (first 12 months)
  • Successful launch or meaningful expansion of at least two integrated care initiatives
  • Clear, actionable project plans, go-to-market strategies, and financial models developed for new offerings
  • Measurable adoption of new offerings across the care center network
  • Demonstrated improvement in patient care and experience
  • Sustainable financial performance of new service lines

Education & Experience
  • Bachelor’s degree required
  • Master’s degree in Healthcare Administration, Business Administration, Public Health, or a related field preferred
