POSITION SUMMARY: In cooperation with Nursing Administration, the weekend Wound Care Nurse position is responsible for the clinical oversight of skin integrity and wound healing throughout the nursing facility during scheduled weekend shifts. The position requires advanced clinical knowledge of wound etiology and treatment techniques. The work requires administrative ability, knowledge of training techniques, ability to adapt techniques and procedures in accordance with unusual situations and the exercise of initiative and good judgment.

RESPONSIBILITIES/ ACCOUNTABILITIES:

  • Implements specialized geriatric and chronic care nursing principles specifically related to wound prevention and management;

  • Performs comprehensive skin checks on residents daily during scheduled shifts to ensure early identification of pressure injuries or vascular changes;

  • Carries out physician-ordered wound treatments and ensures the appropriate specialty surfaces are in place;

  • Reviews weekend admission and re-admission charts specifically for skin history, nutritional status, and initial staging of any existing wounds;

  • Conducts bedside instruction for weekend floor staff regarding proper positioning, offloading, and treatment pass techniques;

  • Monitors the use of preventative equipment (e.g., air mattresses, cushions, heel protectors) throughout the facility;

  • Ensures that individual nursing care plans specifically address wound interventions and prevention strategies;

  • Provides education to residents and families regarding wound status, treatment goals, and discharge/care preparation;

  • Reports all new facility-acquired pressure injuries immediately to the Director of Nursing Services (D.O.N.) or supervisor on duty;

  • Oversees charting and documentation process for each resident with skin-related needs during the weekend shift;

  • Assumes responsibility for the identification of potential safety hazards and for the identification of noncompliance with facility guidelines as set forth in the Policy and Procedure Manual;

  • Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights;

  • Performs other duties as requested.

SPECIFIC EDUCATIONAL/ VOCATIONAL REQUIREMENTS:

  • The Wound Care Nurse must be a graduate of a state-approved school of nursing and currently registered as a registered nurse in the state.

  • Specialized wound care certification (e.g., WCC, WOCN, CWS) is preferred.

  • Up-to-date physical assessment skills and comprehensive knowledge of skin and wound management are required.

  • Recent experience in a long-term care or rehabilitative setting focusing on wound care is strongly preferred.

JOB SKILLS:

  • Must have thorough knowledge of nursing theory, technique and practices.

  • Must have thorough knowledge of the specialized procedures applied and equipment used in the area to which assigned.

  • Must have knowledge of dietetic, sanitation and personal hygiene.

  • Must have knowledge of medications, wound treatments, their proper dosage and expected results.

  • Must have skill in handling and manipulating instruments and wound care equipment.

  • Must be able to coordinate and oversee the wound care functions in an effective manner.

  • Must be able to maintain an empathetic attitude toward residents and to gain their confidence in order to work with them effectively.

  • Must be able to understand and follow oral and written instructions of a technical nature.

  • Must possess the ability to positively interact with personnel, residents, family members, visitors, government agencies/personnel and the general public.

  • Must provide annual verification of a negative TB skin test.

PERFORMS RELATED DUTIES:

  • Interacts with residents, families, visitors and employees.

  • He/she carries out other tasks as requested in situations where hands-on intervention/participation may be required.
