We are looking for a compassionate, skilled RN or LPN to join our assisted living team. You will provide direct nursing care and wellness oversight to our residents, support their independence and dignity, and serve as a clinical resource for our care team. - Conduct resident wellness assessments and monitor changes in condition
- Administer medications and treatments per physician orders and ALF regulations
- Develop and maintain individualized service plans in collaboration with the care team
- Communicate changes in resident condition to physicians, families, and the care team promptly
- Supervise and delegate tasks to medication aides and care staff on your shift
- Perform skilled nursing tasks including wound care, treatments, and catheter care
- Coordinate with physicians, home health, hospice, and outside providers as needed
- Ensure compliance with state ALF regulations and community policies
- Document care accurately and timely in the electronic health record
- Participate in care planning, quality improvement, and resident satisfaction initiatives
- Current, unrestricted RN or LPN license in the state of practice
- Graduate of an accredited nursing program
- Current BLS/CPR certification
- Assisted living, long-term care, or geriatric nursing experience preferred
- Strong assessment, communication, and organizational skills
- Genuine compassion for elderly residents and their families
- Ability to work rotating shifts, weekends, and holidays
