Pay Information

Base pay range: $70,500 – $96,900 per year.

Additional per‑visit compensation: $45–$63 per visit/unit.

Make a meaningful impact by providing personalized, one‑on‑one care to help patients regain independence k closely with a dedicated team of physicians and clinicians to develop and manage care plans that support recovery and help patients return to the life they love.

Responsibilities
  • Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
  • Administer ongoing care and case management for each patient, providing necessary follow‑up as directed by the Clinical Manager.
  • Consult with physicians to develop initial treatment plans based on orders and assessment; provide hands‑on care and teach patients under Clinical Manager's supervision; revise plans as needed.
  • Coordinate care with other healthcare personnel such as physical therapists, occupational therapists, home health aides and external providers.
  • Report patient care, condition, and progress to the patient's physician and Clinical Manager continuously.
  • Implement patient care plans jointly with patients and families to achieve optimal resolution of needs.
  • Supervise, oversee, and coordinate the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers; provide written instructions that reflect the current plan; monitor documentation compliance.
  • Discharge patients after consultation with physician and Clinical Manager, preparing and completing required clinical documentation.
  • Prepare comprehensive medical documentation, including case conferences, medication order changes, re‑certifications, progress updates, and care plan changes; produce visit/shift reports and summarize patient records; collaborate with other health disciplines.
Qualifications & Skills
  • Diploma, Associate, or Bachelor’s degree in Nursing.
  • At least one year of nursing experience (preferred).
  • Strong experience in medical‑surgical, ICU, ER, and acute care settings.
  • Home health experience a plus.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver’s license, auto insurance, and reliable transportation.
