Nurse Practitioner

As a Nurse Practitioner in Waco, TX, you’ll provide focused care in nursing facilities helping reduce readmissions and improve recovery while enjoying autonomy, a predictable schedule, and strong team support.

• Round in assigned facilities on a consistent weekday daytime schedule
• Oversee daily patient interactions through timely micro visits that support focused assessments to assess recovery, manage pain, wounds, and functional decline
• Collaborate with physicians, therapists, and facility staff on individualized recovery plans
• Use Puzzle’s EHR to complete documentation (often remotely from home)
• Monitor readmission risk and help close gaps in post-acute care
• Maintain compliance with post-acute and PM&R-related regulations

• Schedule: Weekday daytime hours only; no nights, weekends, holidays, or call
• Compensation: Competitive salary with the final offer dependent on experience, location, and other lawful factors. Performance bonus and/or RVU-based incentive opportunities may be available

• Master's degree in Nursing and current certification as a Nurse Practitioner (NP) from an accredited program.
• Active, unrestricted NP license in the relevant state(s) of employment, with prescriptive authority.
• Minimum of 2-3 years of experience in PM&R, geriatrics, post-acute care, or a related field; experience in nursing facilities strongly preferred.
• Strong clinical skills in assessing and managing barriers to recovery, including rehabilitation, pain, and chronic conditions.
• Excellent communication and time-management abilities to handle brief, efficient patient encounters.
• Proficiency with EHR systems and remote documentation.
• Ability to work independently with a high level of autonomy and consistency.
