Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• 1-2 years of experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance

Roles & Responsibilities:

• Administer skilled nursing care for clients of all ages in their place of residence.
• Assess home health and hospice patients in stable and emergent situations to identify their needs.
• Develop, document, and evaluate the Plan of Care (POC) to ensure consistent quality care.
• Educate patients and/or their families regarding care procedures, medications, and home safety.
• Supervise and provide clinical direction to home health aides to maintain care standards.
• Perform on-call responsibilities and provide support to patients and families as required.

Compliance / Notes:

• Must comply with all infection control measures.
• Participation in interdisciplinary team meetings as necessary.
