Overview

Licensed Vocational Nurse, Home Health

Location: Waco, TX – coverage area includes McLennan Counties and surrounding areas, focus on 76710.

Position: LVN / FT

Schedule: M-F 8 to 5;
On Call monthly.

Compensation: $36.00 to $43.00 per hour (per‑visit basis).

Under the supervision of an RN, the Licensed Vocational Nurse (LVN/LPN) provides direct patient care to pediatric, adolescent, adult, and geriatric patients. The role includes observing, documenting medical progress, and supporting nursing status changes under RN direction.

Qualifications
  • Graduate of an accredited practical or vocational nursing program.
  • Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation.
  • At least one year of LVN/LPN work experience.
  • Ability to provide nursing visits per physician treatment and maintain accurate medical records.
  • Valid CPR certification while employed in a clinical role.
  • Licensed driver able to travel to all business locations.
Responsibilities
  • Perform technical nursing functions and direct patient care consistent with the established plan of care under the supervision of an RN.
  • Promote and maintain optimal client care under RN supervision.
  • Maintain a safe patient environment.
  • Identify and communicate significant abnormalities in patient data to case manager and other home health team members.
  • Administer prescribed medications and treatments as permitted by law; assess all medicines to identify potential adverse reactions, allergies, contraindications, and report any problems to the physician.
  • Re‑evaluate client’s nursing needs and report findings to the Case Manager.
  • Provide counseling and instruction to the client and family regarding nursing and related needs.
  • Prepare clinical and progress notes.
  • Submit all documentation in a timely manner in accordance with agency policy.
  • Ensure documentation on the patient chart correlates with the nursing care plan.
  • Communicate frequently and work closely with other home care team members.
  • Assist in coordinating services provided to the client.
