Requirements:

• Current state license as a Registered Nurse
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• Homecare Homebase Software Experience
• OASIS Experience
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance

Roles & Responsibilities:

• Assess patients in home health and hospice to identify their physical, psychosocial, and environmental needs.
• Develop, document, and evaluate the Plan of Care to maintain quality and continuity of care.
• Educate patients and their families regarding care, medications, and home safety.
• Supervise and provide clinical direction to home health aides to ensure quality service delivery.
• Monitor assigned cases to ensure compliance with third-party payer requirements.
• Serve as a liaison between patients, families, and the healthcare team.
