Under the immediate supervision of Coordinated Specialty Care Program Manager, the Family Partner performs advanced human services support work in the Heart of Texas Behavioral Health Network offices and community cifically, this worker provides information to caregiver(s) of Coordinated Specialty Care First Episode Psychosis (CSC‑FEP) clients, as well as providing emotional support, experienced guidance, and problem‑solving assistance in a effort to strengthen the caregiver(s) and client relationship.
Examplesof Work Performed
- Per referral from Case Managers, Therapists, Skills Trainers, and/or other Treatment Team members, Family Partner meets with parents/caregivers of clients served in the CSC program to provide experienced advice and guidance concerning CSC services and mental health services in general.
- Under the supervision of CSC Program Manager, provides emotional support to parents/caregivers of CSC clients struggling to meet their unique parental demands and personal challenges.
- Participates as a team member in conferences designed for the diagnosis and treatment of clients.
- Assists parents in identifying barriers and problems as well as in problem solving.
- On behalf of parents/caregivers, acts as liaison and advocate with appropriate agencies, physicians, schools, employers, etc.
- Consults and meets with other KCF Family Partners regularly.
- Co‑facilitates Parent Support Groups in an effort to support, empower, and guide parents of CSC clients.
- Increases knowledge and skills through attendance of outside conferences and seminars whenever possible.
- May provide transportation to clients or caregivers.
- Performs related work as required.
- Excellent written communication and computer keyboarding for inputting clinical documentation
- Excellent verbal communication with clients, staff, and community providers
- Connecting families with resources
- Providing emotional support and guidance to families
- Identifying family goals
- Time management required to meet service goals
- Local travel using Center vehicle
- Stooping, bending, sitting, standing, keyboarding for prolonged periods, and lifting up to 15 lbs.
- Ability to be CPR and PMAB certified
- Valid driver's license and liability insurance required. 25% local travel.
- High school diploma or GED required
- Experience parenting at least one child with a Severe Mental Illness required
- Experience in social services work preferred
Salary: $15.00 hourly / $31,200.00 annually
Hours:
Mon‑Fri 8am‑5pm
