RN Case Manager​/Nursing Supervisor - Home Health Agency - Part-Time Potential

Overview

JOB TITLE: RN Case Manager / Nursing Supervisor

This hybrid role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews.

• RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
• RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
• RN to Document/Communicate/Coordinate patient status/change in condition to care team.
• RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
• Willing to travel surrounding counties.
• Minimum of one (1) year of nursing supervisory experience. Home care experience preferred.

• Graduate of an accredited School of Nursing.
• Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
• CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
• 1 year Experience in case management and home health required, with 2+ years of home health experience highly preferred.
• Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
• Willingness to travel throughout all stated counties in this service area to care for clients is required.
• Strong background in nursing, patient care, and confidentiality.
• Strong attendance and dependability track record with previous employers.
• Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
• Familiarity with HIPAA regulations and medical terminology.
• Current Driver's License and reliable transportation.

• SERVICE AREA: 4+ county radius in North East Ohio, Includes Summit, Medina, Cuyahoga, Tuscarawas, and surrounding Counties, primarily aligned along I-77.
• Typical travel distances require a 30-90 minute daily radius (or less), with occasional travel up to 2 hours to cover all county cases also required.

• JOB LOCATION OPTIONS: Medina County is centrally located to serve all counties in this service area with minimum travel, but qualified applicants residing in or adjacent to any of the defined counties will be considered so long as travel requirements are fulfilled.
• Service Area Counties include:
  Summit, Medina, Cuyahoga, Tuscarawas, primarily aligned along I-77. Additional Counties where RNs may reside to serve the counties above include:
  Stark, Wayne, and Holmes.

• SCHEDULE TYPE: Flexible and customizable. We’ll build a schedule as close to home as possible - that fits your availability and preferences.
• Typical hours are between Monday - Friday (weekdays) during the day (8am - 5pm).

• EMPLOYMENT TYPE:
  Part-time + Long-term employment*
• No On-Call requirements / No Overnight shifts required.
• HCN will expand this role into Full-Time (choice of hourly or salary) for the right candidate.
• Weekend, Overtime, Respite & PRN services are offered when available but are not required.
• Part-Time RNCMs receive hourly compensation (paid weekly, on Fridays).

• Part-time Hourly Wage: $35 - $40/hour

Job Summary:

As an RN Case Manager / Nurse Supervisor, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.

• With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
• Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.
• You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
