Job Summary
The Nursing Assistant (NA) serves as caregiver to residents during scheduled work periods in long‑term care, providing resident‑centered nursing care and daily living assistance under the supervision of a registered nurse (RN). The NA delivers age‑appropriate care for the physical and psychological needs of residents as per the individualized care plan, including basic health‑care needs such as bathing, toileting, grooming, dressing, vital‑sign monitoring, and psychosocial support.
Key Responsibilities- Assists residents with transferring, repositioning, and walking using correct transfer techniques and equipment, and performs range‑of‑motion and passive exercises.
- Prepares food and assists with meals and snacks, providing water and supplements as appropriate.
- Documents resident interactions and outcomes related to care, behavior, activity, and diet.
- Communicates changes in resident condition and care‑related concerns to the charge nurse.
- Collects non‑invasive body‑fluid specimens and gathers vital signs.
- Provides housekeeping and laundry services to maintain resident living areas.
- Assists with wellness and recreational programs, encouraging resident participation.
- Participates in departmental or organizational committees and task forces as needed.
- Adheres to infection prevention and control policies and proper use of personal protective equipment.
- High school graduate or GED (preferred); minimum age 16.
- Prior clinical or nursing assistant experience in long‑term care preferred.
- Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing within four months of start date; training and certification provided by the facility if not already obtained.
- Satisfies yearly department‑specific competency testing and maintains a record of continuing education credits each year; a minimum of 12 hours of continuing education annually.
- Maintains all department‑specific required certifications.
- When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
- May classify as a Sanford Category II Driver, requiring a motor‑vehicle report and proof of a valid driver’s license; must be medically qualified to perform essential driving functions as per Sanford Driving Policy and leadership request.

Starting rate: $19.00–$25.50 per hour; position may qualify for a $1,250 sign‑on bonus.
