- Provide direct nursing care to residents in the LTC unit, including medication administration, assessments, and ongoing monitoring.
- Develop and update individualized care plans in collaboration with the interdisciplinary team; document outcomes in the electronic medical record.
- Administer prescribed medications and treatments following protocol and physician orders.
- Monitor residents for changes in condition, respond to clinical needs, and elevate concerns per policy.
- Collaborate with physicians, therapists, and other staff to coordinate care and support discharge planning as appropriate.
- Active Hawaii RN license
- Current BLS certification
- Minimum of one year of recent Long-Term Care or skilled nursing experience
- Experience with electronic medical records
- Ability to work the assigned schedule, including weekends and holidays
- Availability for all shifts
