• Provide direct nursing care to residents in a Long-Term Care unit, including medication administration and monitoring
• Conduct assessments, document findings, and contribute to care planning
• Utilize electronic medical records to maintain accurate and timely patient information
• Coordinate with physicians and the interdisciplinary care team to adjust plans of care as needed
• Monitor resident status, respond to changes, and ensure safe, quality care in a fast-paced environment
• Communicate clearly with residents, families, and team members to support continuity of care

• Active Hawaii LPN license
• Current BLS certification
• Minimum of one year of recent Long-Term Care or skilled nursing experience
• Experience with electronic medical records
• Ability to work the assigned schedule, including weekends and holidays as needed

• Holiday pay eligibility for select holidays based on hours worked
