Position:
Registered Nurse Case Manager (Full‑time days)
Territory:
Pepperell, Dunstable, Tyngsboro, Westford, Groton, Ayer, Littleton, Acton, Concord.
Compensation: $94,100 to $115,000 annually. Sign‑on bonus: $10,000.
Responsibilities- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other disciplines as indicated by the patient’s needs or document rationale for not doing so.
- Promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning self‑care techniques.
- Supervise LPNs and HHAs.
- Complete documentation timely and accurately.
- Communicate patient progress to the clinical manager and care team regularly.
- Plan and provide staff education.
- Perform on‑call responsibilities and provide on‑call services to patients/families as assigned.
- Participate in clinical development and continuing education programs.
- Perform other duties as assigned.
- One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year, candidate must be approved by VP Clinical.
- Current RN license specific to the state(s) of assignment.
- Current CPR certification.
- Valid driver’s license, reliable transportation and liability insurance.
- If less than 6 months clinical experience as an RN, candidate must participate in the RN Intern program.
- Choice of PPO or HSA medical plans with free telehealth.
