Intensive Family Support Coordinator

Communitas, Inc. seeks an Intensive Family Support Coordinator to empower families of individuals with disabilities. In this role, you will provide individualized support, coordinate community resources, and facilitate family-centered planning to promote inclusion and stability  will collaborate with families, service providers, and community partners to develop and oversee tailored support plans, monitor progress, and advocate for needed services.
