Communitas, Inc. seeks a Specialized Family Support Coordinator to partner with families of individuals with intellectual and developmental disabilities. In this mission-driven role, you will complete intakes and needs assessments, create person-centered support plans, and link families to community resources, respite, and benefits. You will collaborate with schools, healthcare providers, and state agencies, maintain accurate documentation, and facilitate family meetings or workshops.

Responsibilities
- Coordinate specialized family support services for individuals with intellectual and developmental disabilities (IDD) and their caregivers
- Conduct intake, needs assessments, and ongoing service planning with families
- Connect families to community resources, benefits, respite, and advocacy supports
- Collaborate with internal programs, schools, healthcare providers, and state agencies
- Maintain accurate documentation, service notes, and reports in compliance with regulations
- Provide education and guidance to families on navigating IDD systems and services
- Facilitate family meetings, workshops, and support groups as needed
- Monitor service effectiveness and adjust plans based on family feedback and outcomes
- Advocate for person-centered, inclusive supports that promote independence and dignity
- Participate in team meetings, trainings, and continuous quality improvement efforts
