Position Overview
Registered Nurse Home Health – 1061897 at Home Call, a part of LHC Group. This full‑time role involves providing and directing nursing care to patients in their homes and coordinating care with physicians, agency personnel, patients and families. $2,500 sign‑on bonus available for full‑time external new hires.
Primary Responsibilities- Initiate, develop, implement and revise care plans in collaboration with physicians and other health‑care professionals.
- Supervise care provided by aides and licensed practical/vocational nurses, providing instruction and assigning tasks per state and federal regulations.
- Provide required supervisory visits.
- Deliver high‑quality clinical services within scope of practice and infection control standards, documenting patient visits per policy and payer requirements.
- Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
- Make initial and/or comprehensive nursing evaluation visits, ensuring eligibility and medical necessity, and submitting accurate documentation.
- Communicate relevant information timely and effectively with agency staff, physicians, patients and families.
- Follow direction from Clinical Director and Executive Director and complete assigned tasks timely.
- Assist in the orientation of new personnel and serve as a preceptor.
- Participate in survey readiness activities, performance improvement plans, and on‑call and weekend rotations as needed.
- Adhere to and participate in the agency’s utilization management model.
- Current and unrestricted RN licensure in the state of practice.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Ability to function in any home situation regardless of client characteristics.
- Ability to work independently.
- Solid communication, writing and organizational skills.
- Annual total cash compensation 64,100 – 141,500 (based on full‑time employment and productivity).
- Per visit point pay 36.98 – 81.63.
- Hourly rate 30.82 – 68.03.
