About This Role
As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes as prescribed by the physician, ensuring compliance with applicable laws and agency policies. This role is ideal for those who are passionate about patient care and enjoy working collaboratively with healthcare professionals.
Highlights- Pay: $64,100 - $141,500 annual total cash target pay
- Location: Waldorf, MD
- Sign-On Bonus: $2,500 Sign-on Bonus available for full-time external new hires
- Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
- Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
- Provide required supervisory visits.
- Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
- Document patient visits per policy and payer requirements, syncing timely per LHC policy.
- Make initial and/or comprehensive nursing evaluation visits to ensure patients meet home health eligibility and medical necessity guidelines.
- Communicate relevant information timely and effectively with appropriate agency staff.
- Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
- Participate in on-call and weekend rotation as needed to meet patient needs.
- Adhere to and participate in the agency's utilization management model.
- Current and unrestricted RN licensure in state of practice
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
