Entrust Community Services is seeking a compassionate and detail‑oriented Case Manager to support individuals and families across multiple programs. This role provides comprehensive case management services—including planning, coordination, advocacy, and ongoing support—to help reduce barriers and promote long‑term stability and success.
The Case Manager will work collaboratively with Intake and Outreach staff, internal teams, and external community partners to develop individualized plans and ensure all services are delivered in alignment with program requirements and Entrusts mission.
About the RoleThe Case Manager establishes supportive, professional relationships with individuals and families, assists with developing individualized service plans, monitors progress, and connects individuals with community resources. This role requires strong communication, problem‑solving abilities, and a commitment to advocacy.
Key ResponsibilitiesService Delivery & Case Planning
- Partner with Intake/Outreach staff during intake and assessment processes.
- Develop and implement individualized service plans for individuals and families.
- Perform periodic reassessments and update service plans as needed.
- Maintain regular communication and supportive relationships with individuals, families, and service providers.
- Document all case management activities, progress updates, and changes in program databases.
- Distribute completed plans, forms, and reports to internal team members and external agencies.
- Establish cohesive partnerships with Intake, Outreach, and all internal/external service partners.
- Communicate with the Program Manager and team regarding participants awaiting services or transitions.
- Coordinate coverage of services during staff absences to ensure continuity.
- Attend meetings, trainings, and team collaborations as required.
- Research and identify community resources to support individual and family needs.
- Advocate for participants to ensure access to services, rights, and supports.
- Maintain professional working relationships with agencies, care providers, and community systems.
- Exit participants according to program policies and procedures.
- Assist with program-related tasks as assigned.
- Ensure compliance with all program rules and documentation requirements.
Required
- High School diploma or GED and at least 3 years of case management experience, or
- Associate Degree in a related field and 2 years of experience, or
- Bachelor's Degree in Psychology, Sociology, Special Education, or related field and 1 year of experience.
- Valid Washington State driver’s license and proof of insurance.
- Ability to pass a Washington State Criminal Background Check, E‑Verify, and mandatory drug screen.
- Current First Aid/CPR certification.
- Knowledge of service planning, human service systems, and advocacy practices.
- Ability to develop comprehensive plans and technical reports.
- Strong written and verbal communication skills.
- Ability to interpret technical reports and instructions.
- Ability to research community resources and navigate external systems.
- Skill in documentation, relationship building, and maintaining professional boundaries.
- Ability to work both independently and collaboratively in a team setting.
- Proficiency in Microsoft Office and program databases such as Setworks.
