They establish and maintain multiple patient relationships, coordinate care, and support healthcare delivery to ensure comprehensive, patient-focused services.

The role involves engaging with caregivers, enhancing patient interactions, and promoting whole-person care.

• Manage patient panels and coordinate care plans
• Establish and maintain strong patient-provider relationships
• Support primary care team operations and patient engagement
• Ensure adherence to care protocols and documentation standards

• Office experience including customer service, computer literacy, and familiarity with Microsoft Office
• Required:
  
  BLS certification at hire;
  Medical Assistant registration within 60 days
• Preferred: 2 years healthcare experience

• Strong communication and organizational skills
• Patient-centered approach and ability to collaborate effectively
