What You'll Do as an RN Case Manager

  • Complete home health intake assessments, medication reconciliation, and care plans
  • Collaborate with physicians, caregivers, and interdisciplinary teams
  • Educate patients, families, and caregivers on disease management, safety, and recovery
  • Maintain accurate clinical documentation and communication with staff

Qualifications

  • Current RN license in Washington State
  • Minimum 2 years nursing experience; home health experience preferred but not required
  • CPR certification
  • Licensed driver with reliable, insured vehicle
  • Strong communication, clinical judgment, and patient-focused care skills
